Chronic disease management

Living with a chronic disease or condition can be challenging. Staying as healthy as possible requires consistent medical care. The providers at Novant Health Primary Care Associates Madison regularly care for adults and children with chronic conditions, including:

  • Diabetes.
  • Hypertension (high blood pressure).
  • High cholesterol.
  • Asthma.
  • ADHD.
  • Heart disease.
  • Arthritis.
  • Osteoporosis.

Care for these conditions can include education, medication management, help with lifestyle modifications, and referrals to and coordination with specialists in these fields when needed.

Coumadin clinic

Our providers also offer a Coumadin clinic for patients on this common blood thinner. Coumadin, or warfarin, is prescribed to prevent or lessen the severity of blood clots, as well as to prevent heart attacks and strokes. Regular monitoring including blood clotting tests is necessary while on this medication. Our providers can complete needed tests and monitoring, as well as ensure you are on the correct dose of the medication.
